Prednizone- the correct spelling is "prednisone"- is a commonly used oral glucocorticoid medicine.

The adrenal glands manufacture a natural form of glucocorticoid. Glucocorticoids are responsible for many functions in the body including maintenance of blood pressure, proper use of sugar, protein, and fat metabolism, response to stress, and many other tasks.

Glucocorticoids manufactured by the body are referred to as endogenous steroids- meaning a person's own body makes these steroids.

When steroids are taken in from the outside either by mouth, intramuscularly or intravenously, they are referred to as exogenous steroids. Taking glucocorticoids orally or intravenously can reduce the ability of the person's own adrenal glands to continue to manufacture glucocorticoids.

Without the ability to increase steroid production in the face of stressors such as injury, infection, and surgery, a patient can go into shock.

The chances of the adrenal glands being suppressed increase as the dose of "outside" steroid exceeds the average daily equivalent output of the adrenal glands which is about 5.0-7.5 mg prednisone, therapy continues for more than a few weeks or months, doses are given late in the day or in split doses, or long-acting corticosteroid preparations are used.

Patients who require high doses of prednisone (more than 20 mgs a day) for extended periods of time often will develop side-effects.

Taking steroids on an alternate day (every other day) schedule lessens the chance of adrenal insufficiency but does not do away with it altogether.

Other side-effects include:

o Increased risk of bacterial or opportunistic infections such as fungi, tuberculosis, pneumocystis carinii

o Elevated blood sugar

o Fat distribution changes leading to moon face, buffalo hump

o Elevated blood lipids

o Aggravation of hypertension

o Electrolyte abnormalities such as low blood potassium

o Fluid retention leading to edema

o Easy bruisibility

o Increased body hair

o Increased sweating

o Purple stretch marks

o Impaired wound healing

o Glaucoma

o Cataracts

o Muscle wasting

o Stomach ulcers

o Pancreatitis

o Accelerated hardening of the arteries

o Osteonecrosis (bone death)

o Psychiatric disturbance

o Insomnia

o Bowel perforation

o Masking of infection

The use of prednisone for different diseases such as arthritis needs to be assessed with the potential benefits vs. the risks. These benefits and risks need to be discussed with your rheumatologist.

Recent university research programs provide us with new discoveries in glycoscience. Receptor sites have various functions in cell to cell communication.

Corrupted signals from CD81 receptors work with CD38 receptors (both on cell surface) to become the major culprits to spread tumors. CD81 and CD38 signals are not just instructing cancer cells to metastasize but, are actually training the cancer cells by suppling some cell body parts.

This is the first report that indicates cancer cell activity is somewhat like that of a virus when it requests another cell to provide its DNA to reproduce another virus. A recent study about this bad information appeared in the journal Cell. Why would "normal cells" cooperate to spread cancer?

When normal cells are truly healthy, the body has a good immune system that summons macrophages and killer T-cells to the battle front to take on the cancer cells head on. When normal cells are not healthy, their communication becomes corrupted. Healthy cells convey a clear message through a well modulated immune system to the macrophage for protection. That protection system summons help to kill the enemy, repair the damage, and carry out the trash. When the immune system is weak, it is unable to defend the cells. When the immune system is over-stimulated, an auto-immune system may prevail where the cells turn on themselves.

Scientist are learning that glycoprotein receptor sites (CD81 example) contain massive amounts of information, not just a few words, but a collection of instructions. The normal cells and the cancer cells engage in a dialogue. CD38 is involved in apoptosis (programming a cell to die instead of divide) and must serve as a co-receptor to induce signaling within the cell. CD38 is known to be associated with T-killer cells. The message should be, "Kill the cancer cell." Instead, the instructions may be, "Kill the (blank) cell." The corrupted message spreads faster than truth where the innocent are blamed and the "normal" are destroyed.

M.D. Anderson Cancer Center in Houston studied CD81 and was able to decrease its expression. While this is remarkable science, we should do all we can to make the cells healthier and proliferate glycoprotein receptor sites which we have learned reduces inflammation and causes growth of new stem cells which are designed to repair damage. Healthy receptors make for healthy cells and healthy cells make for a healthy body.

Coming to mainstream medical diagnostics is the means of monitoring glycoprotein receptor sites on the surface of cells. Most FDA-approved cancer biomarkers are already glycoproteins.

Classical oncology research has sought ways to kill or stop cancer cells. Researchers believe that to inhibit CD81 signals may be the new pathway for cancer treatment. Now, with the new found CD81 knowledge, billions will be spent on research and development of new drugs in an effort to silence the CD81 receptors.

Turning off CD81 so it will not send bad signals, may also silence it from sending good signals. Why not research how to instruct CD81 to send the correct signals instead of corrupted signals?

I think we know how to program the CD81 receptors to send the correct signals instead of corrupted signals. Somebody give us a grant to correct the CD81 signals and perhaps together we can change the world.

The cancer of the lining of the abdominal cavity is called peritoneal mesothelioma. It is not as common as pleural form; it comprises an estimated one fifth to one third of all diagnosed mesothelioma cases. This report of the Surveillance, Epidemiology and End Results (SEER) approximates the number as 54.7% male and 45.3% female, with the age ranging from 65 to 69. The period of latency is shorter for patients exposed in asbestos, the symptoms appearing after 20 to 30 years from exposure; the usual latency period for pleural mesothelioma is 30 to 40 years.

Peritoneal Mesothelioma Symptoms

At the time of presentation, symptoms of peritoneal mesothelioma includes abdominal mass, abdominal pain, enlarged abdominal girth, fluid in the abdomen (ascites), distention of the abdomen, weight loss, fever, anemia, fatigue and digestive disturbances. For a few months before a confirmed diagnosis, some patients even complain of symptoms that are non-specific. Incidentally, peritoneal mesothelioma is found in a percentage of cases once the patient has other health complaints such as hernia, gallbladder or pelvic mass.

Doctors who have experiences in this field have noted that typically, patients have these symptoms six months to two years prior to the diagnosis. Men often come to the doctor complaining of a bulge in the groin (hernia) or around the belly button (umbilical hernia). For women, the first sign of a problem often occurs after a pelvic test when a tumor mass have been discovered.

In later stages of peritoneal mesothelioma, among the symptoms are increased occurrences of blood clots and obstruction of the bowel. There is a noticeable increase on the platelet count for 50% of peritoneal patients but this may be caused by various disorders, so this is actually of little help in the diagnosis. Low albumin level and anemia can also be among the symptoms.

Peritoneal Mesothelioma Diagnosis

Peritoneal mesothelioma has two clinical types which can be differentiated with the help of CT findings, the "dry" type and the "wet". It is classified as "dry" when there are multiple tiny masses or one dominant localized mass and generally little or no ascites. The "wet" type has widespread small nodules, no dominant mass and a presence of ascites.

If fluid is found, the process of eliminating it is through paracentesis; however the analysis of this fluid has limited diagnostic significance. Normally, a definitive diagnosis may be obtained through tissue biopsy.

Staging

As there is currently no staging system for peritoneal mesothelioma, the most popular system for general cancer staging (TNM system) is used. TNM system has the following criteria: (T) status of the tumor, (N) lymph nodes and (M) metastases. Other general categories may also be helpful in order to determine the stage.

Category 1 - with a localized lesion that can entirely be removed (resected)

Category 2 - the disease is located inside the abdominal cavity on peritoneal or organ surfaces where there is a probability of removing as much tumor as possible (debulking)

Category 3 - the disease is located inside the abdominal cavity and invades organs such as liver or colon.

Category 4 - the disease extends outside of the abdominal cavity

Peritoneal Mesothelioma Treatment

In the past years, the effectiveness of surgery alone or intraperitoneal chemotherapy in the treatment of peritoneal mesothelioma were not proven, that is why multimodality treatment is becoming more popular. Debulking (cytoreductive) surgery is removing all or nearly all visible tumors and may be combined with IPHC (Intra-Peritoneal Hyperthermic Chemotherapy), intraperitoneal chemotherapy and radiation.

Removing all tumors is not always probable; the prognosis for survival can then be in accordance with the completeness of cytoreduction which is established by these criteria:

  • Complete cytoreduction

(CC-0): No peritoneal seeding is found within the operative areas

(CC-1): Nodules less than 2.5 cm carries on after cytoreduction which can be penetrated by intra-cavity chemotherapy; thus the process is termed complete.

  • Incomplete cytoreduction

(CC-2) Nodules with sizes of 2.5 to 5 cm carry on after cytoreduction

(CC-3) Nodules that are larger than 5 cm are found; or there is a merging of tumor nodules that are unresectable at any area within the pelvis or abdomen.

Some patients may have widespread disease where surgery is not considered as "potentially curative", palliative treatment may be done by debulking. Take note that specialized treatments should only be administered by seasoned doctors because peritoneal mesothelioma is a rare malignant disease.

Recently, a clinical trial discovered Alimta (pemetrexed) to be effective as chemotherapy agent on treating peritoneal mesothelioma, either on its own or combined with cisplatin or any platinum-based drug. This reflects the previous findings regarding pemetrexed medication of pleural mesothelioma.

Adrenocortical carcinomas are uncommon malignancies that affect the adrenal cortex (outermost layer of the adrenal glands). These glands are found on top of the kidneys, and are responsible for producing hormones which control some body functions. These hormones include adrenaline, noradrenaline (which control heart rate, blood pressure, etc.), steroids, and some sex hormones (estrogen, testosterone).

Occurrence
Its occurrence is noted most on children younger than five years old, and to adults in their thirties and forties. It is found on both males and females. It is rare, as the chance of getting this type of carcinoma is two out of one million.

Causes
The causes of having this type of carcinoma are unknown, but it may be linked on genetics. People with family members affected with this type have been theorized to have increased risk of getting this type.

Signs and Symptoms
As the tumor develops in the gland, some/all hormones that the gland produces also increase in the bloodstream. This increase can lead to several symptoms that may depend on what hormone levels are affected.

Cortisol: Cortisol is a hormone that is responsible for carbohydrate metabolism, and to the body's response to stress. Its increase can produce symptoms which mimic those of Cushing's disease. These symptoms may include:

Buffalo hump: a hump found almost on the nape area of the body, which is usually made up of fat;

Moon face: a change in the patient's face, which produces puffy cheeks, and makes it rounded, resembling that of a full moon;

Virilization: male sexual characteristics become more prominent in both males and females, which causes increased hair growth in some parts of the body, deepening of the voice, and acne.

Aldosterone: Aldosterone is a hormone responsible for salt (sodium and potassium) and water balance in the body. Increase of this hormone can cause symptoms, such as:
Increased thirst: because of increased sodium in the body, and; muscle cramps and weakness: because of electrolyte imbalance.

Diagnosis

Diagnosis of adrenocortical carcinomas will depend on the approach prescribed by the examiner. The physician can use several methods in order to properly diagnose if one has this type of cancer.

Symptom analysis: using the symptoms provided above, the doctor can already suspect that a patient has this type of cancer.

Laboratory tests: using different blood chemistry and hormonal analysis, a physician can see results such as increased blood glucose (because of increased cortisol), and change of hormonal levels from its normal limits.

Radiology: the tumor formed can be visualized through the use of different radiologic examinations, which can include X-ray, CT Scan, and Magnetic Resonance Imaging (MRI)

Pathology: the physician can request extraction of a part of the tumor through different methods, and confirm its malignancy by doing a biopsy.

Treatment
The initial and the only treatment of this type of cancer is through removal of the tumor itself, especially when it has already affected some major blood vessels. If surgery is successful, the chances of living for five more years will be increased to fifty to sixty percent. However, not all of the patients are surgical candidates. If the patients will not be able to undergo surgery, some approaches such as radiotherapy, and radiofrequency ablation will be taken. Medications to control the production of cortisol might be given, as most of the symptoms are caused by increased cortisol.

Adrenal carcinoma is a dangerous and possibly fatal type of cancer. However, knowing Adrenal carcinoma and consulting your physician as early as soon as possible can help in early management and treatment of the disease.

HIV/AIDS is often referred to as a silent epidemic because of its long incubation period. It means the disease may have infected a person for years already without any concrete symptoms. A person who is positive for HIV/AIDS may even infect other persons with this illness if he/she is unaware of his/her own condition.
The infection of HIV can be divided into various phases. However, in most cases, these phases are hard to discern from one another. The health of the infected person is actually dependent on the immune system of the individual which may also dictate the complications that might take place.
These days, HIV/AIDS is considered a chronic disease since an infected person may live for several more years due to the benefits of antiretroviral therapy. HIV/AIDS can be a very deceiving illness since a patient may remain very healthy for a long time with no symptoms at all although the virus is progressively destroying his immune defenses. This is what makes it hard to diagnose early. Wearing high heels has been women’s favorite fashion for past decades. People say heels are bad for the feet and ankle causing aches and pains, may also cause back pain indirectly. Headaches can be a real pain in the neck. They are an almost universal complaint with 90% of Americans saying they’ve experienced a least one bout of head pain in their past. But what if the headaches keep coming back or never really go away? Approximately 1 in every 6 individuals or 45 million people in the United States suffer from chronic headaches. Also known as “cephalgia,” headaches cost billions of dollars in treatment expense and lost worker productivity every year. Headache symptoms which persist and worsen call for a visit to your primary care physician to begin the process of diagnosis and treatment.



There are a number of different ways to classify headaches. They can be referred to as either secondary or primary. Secondary headaches occur as a result of some other condition such as a stroke, infection, brain tumor or head injury. Primary headaches, on the other hand, occur when the headache problem is the disorder. These can be further categorized as one of three main types: vascular, muscle tension and cervicogenic.



The most common type of vascular headache is migraine. Migraine headaches are usually characterized by throbbing pain on one or both sides of the head with a combination of nausea, vomiting and sensitivity to light and/or noise. About 15% of migraine sufferers experience an aura such as vision changes or disruption in taste, smell or touch as a precursor to an attack. While circulatory changes occur during a migraine, the cause is actually thought to be chemically mediated and neurologic in nature. According to nurse practitioner Penny Bernards, a headache specialist at the Neuroscience Group of Northeast Wisconsin, about 40% of migraines affect both sides of the head. Almost three times as many women as men are afflicted by migraines and 60% of the time there is also complaints of neck pain. Being alert to physical or environmental triggers such as certain foods, hormonal patterns and stress will help develop individualized treatment strategies and assist in determining if and what type of medication should be prescribed in your case.



Tension headaches are typified by a constricting sensation or “vice-like” ache felt in the head, neck and jaw musculature. Contributing factors may include anxiety, fatigue or unresolved anger. Caffeine and nicotine intake also may play a role. Tension headaches have been correlated with myofascial pain dysfunction in which one frequently experiences pain in the temples and in front of one or both ears which tends to worsen with chewing, yawning or talking. Mouth opening range of motion may be limited or out of balance. Treatment is aimed at identifying and extinguishing muscle guarding and faulty movement patterns. Inform your dentist or oral surgeon if you find yourself grinding and clenching your teeth. Massage, yoga and physical therapy may help with relaxation training.



Cervicogenic headaches (CGH) originate in the neck and involve the anatomical structures innervated by C1 to C3, the top three neck vertebrae. The International Headache Society has established criteria for diagnosing CGH: Pain is typically located at the base of the head and may project to the forehead, orbital (eye) region, temples, vertex (top of the head) and ears. These headaches are precipitated and aggravated by neck movements or sustained neck postures. Painful limitation or asymmetry in upper cervical motion along with changes in the neck muscle contour, texture, tone or response to stretching is commonly seen. 50% of CGH are related to trauma and may be accompanied by dizziness or difficulty swallowing. A skilled manual physical therapist is trained to find and treat movement restrictions associated with cervicogenic headaches. Studies have shown that manipulative therapy and exercise can reduce the symptoms of CGH and these positive effects are maintained over time.



Thankfully less than 5% of all headaches are considered potentially life threatening. The National Headache Foundation (www.headaches.org) does advise you to consult your healthcare provider if your headache is sudden and severe, occurs with a fever or includes significant neck stiffness with uncontrollable vomiting. If you are over 50 years of age, your headache occurs after trauma or includes bouts of confusion or a loss of consciousness; prompt medical evaluation is strongly encouraged. Any headache that is accompanied by numbness, weakness or vision loss should be looked into. In general, any headache which clearly interferes with your ability to function normally at work, rest or play warrants further examination.
